You and your doctor discuss and decide what your care will include. You know what your care will cost.
Whether you choose Visit-Based or Full-Access Care, your relationship with UP Holistic Medicine will be geared for maximum value and personal care, centered around you.
Patients of all ages are welcome at UP Holistic Medicine, where we offer full-scope family medicine, including primary care, annual wellness exams, women’s and pediatric health, and collaborative care with other health professionals and specialists.
VISIT-BASED CARE
Visit-Based Care is a popular, fee-for-service option, to interact with UPHM. Each visit to the office is an individual charge, based on the length of the appointment and complexity of the visit. As a more "a la carte" health care option, Visit-Based care can include office visits, as well as phone/text/email/Zoom access.
FIRST VISITS are important in building a relationship with our office. After completing an extensive 42-page health questionnaire prior to your office visit, you will then have an hour-long appointment with your doctor to review your concerns and options, and prioritize your plans.
FOLLOW-UP VISITS may include nurse visits for lab work or IV therapies, and/or visits with your doctor to review labs, manage ongoing health issues, and evaluate your changing concerns.
ACUTE CARE VISITS may include evaluation and treatment options for “acute” issues, such as sore throats, colds/sinusitis, ear pain, cough, flu/influenza symptoms.
FULL-ACCESS CARE
Full-Access Care involves membership at a level where you have access to your doctor, 24/7, for phone/text/email/Skype as well as office or home visits (within 15 miles of our office). This “Full-Access” package includes 8 office visits per year, unlimited emails, and emergency texting. This option is ideal for people who: see their doctor more than 2-3 times per year; prefer a more frequent or higher level of connection which the electronic means allow; desire peace of mind knowing their doctor will have time for them whenever they are in need; have a chronic health condition requiring a commitment to care at UPHM that makes financial and logistical sense.
This “Full-Access” package includes 8 office visits per year, unlimited emails, and emergency texting. This option is ideal for people who see their doctor more than 2-3 times per year.
